Summary
The Certified Diabetes Educator – provides comprehensive diabetes education and clinical support to individuals and families affected by diabetes and related metabolic conditions. The role focuses on patient-centered education, self-management training, care coordination, and evidence-based clinical practice to improve health outcomes and quality of life.

Responsibilities
1. Assess patient learning needs and readiness for self-management.
2. Provide individualized and group diabetes education
3. Educating blood glucose monitoring, insulin use, medications, and diabetes technology
4. Support lifestyle modification including nutrition and physical activity
5. Monitor outcomes and prevent diabetes-related complications
6. Collaborating with interdisciplinary healthcare teams
7. Maintain accurate clinical documentation
8. Participate in quality improvement initiatives

Qualifications
• Must hold a full, active license, in good standing with a recognized regulatory body in Ontario as a health care professional (RPN, RN, RD, Kinesiologist).
• Minimum two years of relevant experience
• Strong teaching, communication, and clinical skills
• Certified Diabetes Educator (CDE®) or eligibility
• Experience with interpreting data from continuous glucose monitoring devices is preferred
• Experience and proficiency with insulin dose adjustments for clients on insulin is preferred
• Experience in community or chronic disease management settings
• Patient-centered and culturally competent care
• Critical thinking and clinical judgment
• Proficiency with electronic health records
• Ability to work independently and as part of a team
